Occasional dark image output with tone mapping

Originally reported on Google Code with ID 1495

Describe the problem and how to reproduce it.
With the attached pp3 file, the below-linked raw shows bright in preview but very dark
in image output (links below), only when tone-mapping is enabled. When tone-mapping
is disabled, preview and output image match very closely (I can't tell the difference).

Symptoms similar to #1246, but cause seems different. Not all images do this, and I
even had one image where it did it once, then I restarted fresh and it worked ok.

Tone mapping is known to have some issues (also crashed), however the developer creating
(and being the only one understanding it) it is not active currently unfortuntely..
