Explore moving Windows testing to Appveyor or Circleci

appveyor is a Windows-oriented testing service like CircleCI.

We explored testing on Appveyor a year or so ago, but they did not have the capability to use Linux containers in their docker setup.

According to [forum issue](https://help.appveyor.com/discussions/questions/33953-docker-linux-containers-in-windows-docker-workers] they are about to roll out the latest WIndows Server, which they expect to be able to use to provide this.

    Note that build start is typically about 3 minutes (time to provision VM on Azure).

    Please let us know how it works for you.

    Note that image: Windows Server 2019 is essential to run build on those VMs.
