Playbook Consolidation - Initialization

This is the first in a series of PRs for consolidating the playbooks/ directory structure in OpenShift-Ansible.

The goal of this PR is to consolidate the initialization playbooks from byo/openshift-cluster/initialize_groups.yml and common/openshift-cluster/std_include.yml.

• Initialization playbooks have been moved to playbooks/init/
• initialize_groups.yml has been incorporated into evaluate_groups.yml
• std_include.yml has been moved to playbooks/init/main.yml
• If playbooks were previously only calling evaluate_groups.yml, this has been left the same in this PR
• Ansible tags have been removed from initialization playbooks
• Upgrade playbooks use a piecemeal approach to initialization which will be addressed in a later PR
