Question

I have this page

using an html code I get here.

And I'm getting a error in line 65 of the php code.. the error says

Notice: Use of undefined constant - assumed ' ' in C:\wamp\www\mTest\send_form_email.php on line 65

BTW I don't need a database to do this do I? php code:
