You can try to load the HTML with DOM and then extract the information using xpath:

// create a DOM document object and load your html
$doc = new DOMDocument();
$doc->loadHtml($yourHtml);

$selector = new DOMXpath($doc);

// you may refine it
$result = $selector->query('//form');
